Description

membership/admin/issues.csv should have a page that describes what groups it expects to submit, what "50%" vs. "confirmations" means, etc.. This can also have a checkbox for "include account numbers in output" (ASA-#188), probably defaulting to false.

    * Describe what, exactly, causes the various issue names mean (well, more
      exactly than "50%" supplies...)
    * Provide a version of /membership/admin/issues.csv that includes account
      numbers, for ease-of-fining

Also, joy of joys, discovered that issues.csv generation is buggy, and filed ASA-#191. *sigh*
