Description

It would be sorta nice if the new group form recorded who pressed the "Approve" button somewhere.

    Add note on successful recognition, too (ASA-#213)
    
    We had been adding a note when recognition was rejected. Now we also add a note
    when recognition is granted, including storing the user who hit "Approve".
