LEF/ARCADE app form that doesn't suck

Description
At the moment, the LEF/ARCADE app form errors if you leave things unfilled (I gather?), so we're setting a bunch of defaults. This is lame, especially the bit where we aren't defaulting the cycle, because we know we'll forget to update it. We should write a better app form (possibly in the DB), or fix this one.
